Palliative care is an effective, holistic, multi-disciplinary medical specialty which focuses on easing symptoms, pain, and stress of a serious, chronic illness.  It is appropriate at varying stages of illness and can be provided alongside curative treatment. Palliative care is provided in the home by a doctor, nurse, and social worker. 

This team coordinates care with other healthcare providers, the patient’s doctors, and specialists to provide an extra layer of support. Our team of skilled professionals celebrate life, bring joy, and serve to heal and nurture patients and families.

WHY PALLIATIVE CARE?

Palliative care provides an extra layer of support to you and your family as you cope with serious illness.  This unique type of care is focused on comprehensive symptom management.  The palliative care team will work in partnership with your doctor to help with communication and make sure your questions are answered.  The palliative care team will also ensure your treatment aligns with your health goals while providing counseling during emotionally difficult times.

If you have a terminal or chronic illness, our palliative care physician can come to you to provide an assessment and determine if the palliative care program is right for you.

IS ON MY TEAM?

Your palliative care team will include a doctor, nurse, a social worker and chaplain to ensure your physical, emotional, spiritual and social needs are met. Our team works as consultants with your primary care physician or specialist.

CARE HAPPENS

 Palliative care can take place in a variety of settings.  

Your palliative care team is also available by phone to answer questions and can help when physically getting to the doctor is difficult for you.

The Brazos Valley Palliative Care team is here for you and will respond to your needs with hope-filled care.  Anyone can call to make a referral or inquire about palliative care services.  We also routinely accept referrals from physicians, health care professionals, or assisted living staff, hospital staff, or family members.  Our team of compassionate professionals are available to provide you with
