Transitions is a program administered by Hospice Brazos
Valley, a non-profit, community-owned agency. Transitions
is committed to supporting individuals touched by illness.
Through Transitions, the clients, family and social
worker identify needs and set goals in order to enhance
clients' quality of life. Services are provided to all
people in need without regard to race, age, faith, diagnosis
or ability to pay. The Transitions program is offered
in Bryan, College Station and Brenham. Referrals to
Transitions are welcome from physicians, family members,
discharge planners, and home health agencies.
