Transitions
Transitions is a program developed to provide volunteer and case management services to individuals with a life limiting illness. One of the goals of the Transitions program is to assist the client and family to determine needs and to identify community resources to help with the identified needs. The Transitions program utilizes trained volunteers and community resources to provide and coordinate support to the client and family. Transitions activities are client self-determined and focused on the options available.
Referrals to the Transitions
program are welcomed from physicians, family members, friends, neighbors,
church members, hospital discharge planners or home health agencies. The
clients' physician must approve the client's appropriateness for admission
to the Transitions program. There are no fees associated with receiving
services from the Transitions program. Transitions services are provided
by Hospice Brazos Valley. The programs are made possible because of community
support.
Currently, services of the Transitions
Brazos Valley program are offered in the Bryan/College Station area and
Brenham, Texas.
