Privacy Practices


At Hospice Brazos Valley, maintaining privacy and confidentiality for our patients, families, bereavement clients, and donors is a priority.

We seek to provide the best hospice and palliative care possible to meet the needs of our patients.

As we continually implement technology upgrades to benefit our clients, we will safeguard personal information however we receive it or by whatever means we are contacted.

Hospice Brazos Valley will not share your personal and/or medical information with anyone except as permitted or required by law.

 

Hospice Brazos Valley, Inc. will not sell your personal information.

Information Collection

We collect and use information about those we serve in order to provide hospice and palliative care support, pain control, grief and bereavement support, and other end of life services.

We obtain information about those individuals who donate to our resale stores.

We obtain information about those individuals who participate in our fund development activities.

We obtain most of this information directly from our clients, primarily through the completion of forms when applying for services and through interactions with staff members. If we require additional information from other parties, we will first seek our clients' permission. Examples of other parties may include other medical care providers.

Information Protection

To guard our clients' personal information, we maintain physical, electronic, and procedural safeguards that comply with federal and state laws and regulations.

All of our employees are required to protect the confidentiality and privacy of our client's personal and medical information. Staff may not access client information unless there is a legitimate hospice or palliative reason, such as responding to a client request for service.

Our strict policy about the protection of personal information applies to current and former clients.

Information Disclosure

The Privacy Act permits disclosure of private health information for the following purposes:

  •  Treatment
  •  Payment
  •  Operations
  •  When authorized by the patient
  •  When required by law
   

 

 

 



This may include information that may be needed for use in litigation; information that could endanger the health or safety of the patient, staff, or anyone else; public health purposes; reporting victims of abuse, neglect, and exploitation; information needed by law enforcement; and information needed by coroners, medical examiners, funeral directors, or tissue/organ donation.

By law, we have the right to modify our privacy policy at any time.
