Under
the direction of the patient's physician, the hospice
team delivers specialized care to patients living with
a terminal illness.
Palliative Medical Consultation:
The Hospice Medical Director provides consultation and
back-up services for your primary physician.
Skilled Nursing:
Hospice nurses are expressly trained in pain and symptom
management related to serious and terminal conditions.
Services are provided to patients of all ages, pediatric
through geriatric. Nurses make scheduled visits and
on-call nursing assistance is available 24 hours, 7
days a week.
Personal Care:
Certified hospice care aides attend to the personal
and hygiene care of patients, always mindful of the
patient's dignity and independence. Hospice care aides
make scheduled visits based on the level of care needed.
Social Services:
Hospice social workers help ease the distress associated
with living with a terminal illness by assisting the
patient and family with practical issues. Social workers
identify and facilitate community resources and benefits
available to patients and their families. Social workers
make scheduled visits and are available for consultation.
Spiritual Care:
Hospice recognizes the spiritual diversity
of each patient. The Hospice chaplains and team will
support the patient's and family's spiritual preferences.
Chaplains
may consult with the patient's own clergy as desired
or needed.
Volunteer Support: Hospice trained
volunteers can help caregivers and families. A volunteer
may sit with a patient for an hour or two, allowing
the caregiver to get out of the home, run errands, visit
with a friend or take a nap. Volunteers are assigned
on an as-available, as-needed basis at the request of
the patient or caregiver. Learn

Grief & Bereavement Support:
Grief and bereavement counseling is available to patients,
caregivers and family members. Individual and group
bereavement care programs help survivors through the
