YOU LEAD
WE FOLLOW

There are few areas in healthcare where you are at the center of all decisions.  Your goals are our goals and we are committed to helping you achieve them.  Hospice Brazos Valley's team of hospice care experts are here to help guide you and your family through difficult decisions and regain peace and control.  Your hospice team will include doctors, nurses, hospice care aides, social workers, chaplains, and volunteers all focused on your comfort and walking alongside you every step of the way.

WHEN

IS THE RIGHT TIME

If you are choosing to no longer seek curative  treatment for your chronic or terminal illness, are experiencing poor quality of life, and would prefer care in the home, it may be time to explore hospice care.

WHERE

CARE HAPPENS

Hospice is not a place.  Hospice care can happen wherever you call home.  However, should you require frequent assessment and skilled medical interventions we can do inpatient visits at local hospitals.

WHAT

HOSPICE CARE IS

Hospice provides medical, emotional, and spiritual comfort and support for you and your whole family.

The goals of hospice are to maximize quality of life, provide comfort and help you achieve your goals.

BEGINNING
CARE

Deciding to consult with any hospice team can be a difficult decision.  Hospice Brazos Valley is here for you and will respond to your needs with hope-filled care.  Admission to hospice services often begins with a discussion with your doctor, but anyone can call to make a referral or inquire about our services.  We routinely accept referrals from physicians, health care professionals, nursing home or assisted living staff, hospital staff, or family members.  Our team of compassionate professionals responds quickly and is always available to provide you with step-by-step information and guidance.
